Transitional Care

Each client works with their treatment team to create an aftercare plan that works for them. We have over 2,000 providers in our nationwide network for aftercare placement, which includes all levels of care, in person as well as telehealth, and specialty programming.  Additional programming may include housing/transitional living, life skills training, assistance with re-integration into the workforce, alumni programming, sober support/fellowship meetings, 12-step programs, and other community support activities.
